Around a week ago I started up New Vegas on my second screen, which is a TV hooked by HDMI. I would press play on the launcher and after a few seconds the game dropped to the desktop. I clicked back on the Icon on my task bar and once again it dropped back to desktop. This occurred every time I tried. The game would work just fine on my laptop screen, however. I uninstalled all my mods and attempted to startup again. This time I got an error message saying something was missing and I needed to reinstall. I deleted all content in the New Vegas folder and reinstalled. I turned my mods back on and started the game, not on my second screen but on my main screen. This time the game crashed as soon as it started. Didn't even make it to the disclaimer screen. I uninstalled the mods and tried again. The game continued this trend, crashing immediately on startup every time. I reinstalled again, turned no mods on and still this continues. I am not using Raptr or any other program that messes with displays (as far as I know) The game has been working up until this point.
I use Windows 8.1
Processor - AMD A8-5550M APU 2.10 GHz

Try the game, on new install, WITHOUT ANY MODS installed. Not just activated, but installed also
Steps:
- Uninstall (Not Delete) New Vegas. Do this true Steam, or through your uninstaller manager
- Remove all mods that you own. This should be in the default directory in which FOMM installs the mods
- Redownload New Vegas via steam (no mods)
- Verify Game Cache, see if it brings up errors. If so, let it run and redo this step when done. If not, proceed
- Try the game, on your main laptop (no secondary display). Set resolution out of game to native resolution
- See if the game works. If it does, then close the game, plug into second display
- Set second display to native resolution
- Play game on second display, see if that works
If it does, then TADA. If not, just comment. Remember, don't install any mods until you can do the steps above.

Hide you mods somewhere and remove FOMM. Found in your Program FilesGeMM directory. (if applicable)
Remove Fallout New Vegas.
Delete folder: My DocumentsMy Gamesfalloutnv
Delete folder: UsersxxxxxxappDataLocalFalloutNV
Reboot and clean the registry. (CCleaner or RegMechanic, preferably both)
Reboot and reinstall.
Get it to work on your secondary screen before adding anything. Apply any resolutionvideo mods first, in case you have to repeat from step one. If you are planing to have mods installed, I suggest dis-allowing Steam to update the game and also never let it Verify Integrity of the game.
I have found that when Steam asks for this, complains that a game isn't even installed or if the game doesn't boot, Steam itself needs an update. Quit Steam and watch it update when you restart it. Keep exiting Steam and restarting until it opens without doing another update. A reboot may also be required.

New Vegas Loading Screen Freeze

The game is very buggy, make sure you have the latest patch installed (should happen automatically if Steam is not in offline mode). Also try to install the latest Nvidia driver.
But most likely you just have to wait for a new patch to fix some more of the numerous crashing bugs this game obviously has.

Fallout New Vegas Crash On Loading Screen Windows 10
If you have a multi-core machine, you may want to try these instructions: http://www.pdsys.org/blog/post/2009/02/07/Fallout-3-VideoDisplay-Freezes-sound-keeps-going.aspx
Please Note: Even though this was written with Fallout 3 in mind, it also works for Fallout New Vegas. Enjoy!
The game engine has some issues with processors that have more than 2 cores. You can force the game to only use two of them and it will stop the freezing. I haven't had it freeze once since I did this several days ago.

Open up the fallout.ini file in: My DocumentsMy GamesFallout3
(Note: In Fallout New Vegas, Most tweaks which would normally go in the Fallout.ini file in Fallout 3 actually need to be made in the Fallout_default.ini file found under the Program Files (x86)Steamsteamappscommonfallout new vegas directory in Fallout: New Vegas. This is because when the game is launched via Steam, it opens the launcher first, which overwrites your Fallout.ini file with the contents of the Fallout_default.ini file, along with your launcher settings. Make a backup of Fallout_default.ini, then edit it with a text editor.) (Source)

From the PC Gaming Wiki's 'Fallout New Vegas - Game Crashes Randomly' article:
D3D9.dll Replacement
- Start the installer in compatibility mode for Windows XP SP3 and as an Administrator.
- Download D3D9.dll from MediaFire or a similar source.
- Place D3D9.dll in the directory that contains FalloutNV.exe.
Random Crashes (Random Crashes that tell you FalloutNV.exe has stopped working.)
- Download the following mods: NV Stutter Remover, CASM, Fake Fullscreen Mode Windowed.
- For assistance on downloading these mods: http://www.youtube.com/playlist?list=PL0E5E8CA384409B11&feature=plcp
I have personally tried NV Stutter Remover and CASM. NV Stutter Remover modifies the game to be more 'stable' (more info on its New Vegas Nexus page here). CASM (Cipscis' Automatic Save Manager) allows for an incremental auto-save function specified by a time preset (can also be done manually through a hotkey), allowing you to load a savegame close to when FNV crashed. Fake Fullscreen Mode Windowed runs the game in windowed mode, which some report as helpful in preventing crashes (more info in its New Vegas Nexus page here).

Please help I've been trying to get this to work for a few days. I've tried everything I've looked all around the web to find some fixes for this So far this is what I've tried: New Launcher/FalloutNV.exe d3d9.dll Changing the compatabillity to Windows XP Pack 3 Any ideas what is wrong? Here are my specs: Processor: Intel(R) Core(TM) i5-3210M CPU @ 2.50GHz 2.50 GHz Installed memory (RAM): 4.00 GB (3.82 GB usable System Type: 64-bit Operating System, x64-based processor If you guys need anymore information just ask. I notice that thses errors seem to be quite common even just taking a quick look in this forum and with a google search, but I have the same problems. I don't know if any body knows of any fixes so I just though I should ask in the forum, I've made a tech support query to bethesda too but no reply so far and patch 1.3 has not solved any thing and has introduced new problems. First of all is the crashes, they are random in the time it takes for them to happen from when I first start to play but once it has crashed in a particlar place on re-starting the game often it will happen in exactly the same place again imediatley, the crash just causes the game to close but my pc is fine im just back to steam where I can re-start the game and it will re-start. As of 1.3 update I now have a new problem I didn't have before on exiting or entering an area it can become stuck on the transition loading screen the animated roulete wheel still spins and the back ground changes but the game will never finish loading, I have left it up to half an hour last night whilst I was busy and came back to find it still there so it isn't just a matter of waiting. I don't have any mod's installed, I have tried re-installing the game yesterday and this has not helped I tried deleting the .ini files and having steam check the integrity of the game files this has not solved anything. I don't have the DLC and I won't buy any with the game in this state as I can't play the game as it as. I have owned this game quite a while and personnally I don't see any attempt to fix it being made as two patches have not fixed the crashes and 1.3 brought a new problem which stop me from being able to play but maybe the community has come up with a fix although it isn't the communities job to fix a broken product that never worked right out of the box.I notice that thses errors seem to be quite common even just taking a quick look in this forum and with a google search, but I have the same problems.
